Allergy Related Forms/Plans: If your child has an allergy or requires medicine to be administered at school, please complete the following forms where applicable:
Medication at School Release Form Please complete this form if medication is to be administered at school.
Self Administer Asthma or EpiPen Please complete if your student will be self-administering allergy or asthma medicine on school property. An emergency plan and appropriate allergy card template must also be completed.
Allergy and Anaphylaxis Emergency Plan This plan is REQUIRED if your student has an allergy or asthma condition.
